What it gets you depends on the airline, but it commonly means pre-boarding, keeping the family seated together, and help through the parts of the airport where queueing is the real problem. You do not have to explain a diagnosis to anyone at the airport. The code does that once, quietly, in the system.
Two practical points. It should go into the booking well before departure, and roughly 48 hours ahead is the practical floor for ground staff to act on it. And it has to be honored by whichever airline actually operates each flight, which is not always the airline whose code is printed on your ticket.
